An Interview with Dr. Rebecca Weiss: The 3 Expert-Recommended Treatments for Melasma and Sun Damage

Q: Dr. Weiss, what causes melasma and pigmentation issues?

Dr. Weiss:
Melasma and pigmentation problems are typically triggered by sun exposure, hormonal changes, heat, and inflammation. What many patients don’t realize is that pigment behaves like an iceberg. What you see on the surface is only part of it. There’s often deeper pigment beneath the skin that can resurface over time.

“That’s why treatment and maintenance are both important.”

Q: What is the most important step patients should take?

Dr. Weiss:
“Without question, daily sunscreen use is the most important step.

“I recommend a broad-spectrum SPF 30 or higher, applied every morning and reapplied throughout the day. Mineral or physical sunscreens containing zinc oxide and/or titanium dioxide are ideal, especially for patients with melasma.

“The good news is that modern mineral sunscreens now come in tinted formulations, so they blend naturally into the skin without leaving a white or chalky appearance.

“If you’re not wearing SPF daily, no treatment will maintain long-term results.”

 

Q: What in-office treatments do you recommend for pigmentation?

Dr. Weiss:
There are two primary treatments I recommend, depending on the patient’s skin type and the severity of pigmentation.”

1. 1927nm Laser Treatments (Clear + Brilliant Permea)

“This wavelength laser is excellent for targeting pigmentation and improving overall skin tone. It works by stimulating skin renewal and breaking up excess pigment.

“Most patients benefit from a series of three to six treatments, depending on how significant the pigmentation is.

“It’s a gentle yet highly effective option with minimal downtime.”

2. IPL (Intense Pulsed Light) – Also Known as a Photofacial

“IPL treatments target brown and red discoloration in the skin. The light energy specifically targets excess melanin and visible sun damage.

“Like laser treatments, IPL is typically performed in a series of three to six sessions for optimal results.

“Patients often notice clearer, brighter skin after completing their treatment series.”

 

Q: Is pigmentation permanently gone after treatment?

Dr. Weiss:
Melasma and pigmentation require maintenance. Because pigment can sit below the surface, it may return over time, especially with sun or heat exposure.

“As your skin naturally sheds and regenerates, deeper pigment can rise to the surface. Maintenance treatments and diligent sun protection are key to long-term control.”

 

Q: Can heat worsen melasma?

Dr. Weiss:
Yes, and this surprises many patients. Heat alone can stimulate pigment production, even without direct sun exposure.

“Activities like hot yoga, saunas, and excessive heat exposure can sometimes trigger melasma flare-ups. Patients prone to pigmentation should be mindful of this.”
